Funding Opportunity Announcement - 2015 Tobacco & Cancer Control Community of Practice

Summary

The National Council is pleased to announce a Call for Applications for the 2015 National Behavioral Health Network's Tobacco & Cancer Control Community of Practice (CoP). The CoP is open to any organization that provides direct services to individuals with mental illnesses and substance use disorders. Selected participants will join a 9-month CoP comprised of 10 – 20 direct service provider organizations (e.g., community mental health centers, addiction treatment organizations, federally qualified health centers) seeking to enhance their tobacco and cancer control practices for our target population. Through participation, you will gain knowledge and skills to:

  •  Tobacco-Free Campuses
    • Implement campus-wide tobacco-free policies (addressing both client and staff cessation
  • Implementing Cancer Screening
    • Implement cancer screening (both in integrated care settings and via referrals to local FQHCs)
  • Tobacco Cessation Tools
    • Implement practice guidelines for addressing smoking cessation for youth and adults (including counseling and use of Nicotine Replacement Therapies)
  • Cancer Survivorship
    • Incorporate cancer survivorship into care planning

Resources available to participants will include:

  • Tobacco free policy toolkits
  • Cancer prevention and treatment tools and resources
  • Healthcare professional tobacco cessation toolkits for adults and youth
  • Cancer survivorship care plan tools and resources
  • National expert office hours

PROGRAM OVERVIEW
From January through August 2015, the National Behavioral Health Network's Tobacco & Cancer Control Community of Practice will participate in four (4) learning sessions with national experts available only to CoP participants. The learning sessions will coincide with National Behavioral Health Network webinars that cover issues directly related to topics discussed during the learning session calls. National experts will also hold office hours exclusively for CoP participants who wish to receive more individualized coaching.


During September 2015, participants will report their accomplishments, challenges faced, and changes implemented that resulted from participation via the end of program survey.

PROGRAM EXPECTATIONS & REQUIREMENTS
Participants must be available to engage in the following program activities and produce the following deliverables throughout the 9 months:


  • One (1) customized learning session call every other month, from January through August 2015; four (4) total learning session calls
    • Each site will be expected to share their best practices, lessons learned, and challenges faced on at least one (1) of the four (4) learning session calls
  • Complete the Tobacco & Cancer Control Community of Practice survey at the end of the program (September 2015); the survey will collect information on participant:
    • Accomplishments
    • Challenges faced
    • Changes implemented

The estimated time commitment for participation in this program across 9 months is roughly 9-10 hours (four calls, two surveys, planning for peer-to-peer engagement discussion on learning session calls).

APPLICANT ELIGIBILITY
Organizations are eligible for the Tobacco & Cancer Control Community of Practice if they provide direct services to individuals with mental illnesses and substance use disorders and are seeking to enhance their tobacco and cancer control practices. This program is only open to National Behavioral Health Network participants.
