Seeking Cancer Survivors and Physicians to Participate in a Survivorship Care Plan Pilot Project


Are you interested in developing a way to upload cancer treatment information in a short format? We are recruiting 10-20 physicians from the Orlando and Pensacola areas to participate in this exciting pilot project. The end goal is to electronically generate a summary compilation of a cancer patient’s treatments (diagnosis, medications, surgeries, anesthesia, etc.) and a Survivorship Care Plan. 
 
Physicians: Physicians who participate should have at least five patients who have survived cancer. Physician participants in this pilot will receive:
  • Free subscription to Direct Messaging and a free Graphic User Interface system 
  • SHI will provide software, an e-mail address, and internet interface
  • Free hour of training for a staff member in making the query to generate the summary

We are recruiting physicians who have treated cancer patients to participate in the program now. Participation will be needed from July to September 2015 to test the system and generate Survivorship Care Plans.
